Services available to eligible participants and subject to availability.
Housing and Utilities Support
- Modifications like ramps, handrails, and grab bars to make your home accessible and safe
- Repairs to fixing water leaks to prevent mold growth
- Sealing holes and cracks to prevent pests from entering
- Modifications to air conditioner, heater, humidifier, or dehumidifier to improve ventilation
- Assistance with applications for safe and stable housing in the community
Nutrition and Food Supports
- Registered Dietitian services and group nutrition classes to support you in choosing healthy foods to meet your needs and goals
- Prepared meals, fresh produce, or grocery items – tailored to your specific health needs
- Cooking supplies like pots, pans, small appliances and utensils to prepare meals
Transportation
- Public or private transportation to non-medical places such as: job interviews, parenting classes, housing court to prevent eviction, local farmers’ markets, and city and state department offices to obtain important documents
Care Management
- Helping you find a job or training program, apply for public benefits, manage your finances, and more.
- Connecting you to services like childcare, counseling, crisis intervention, health homes program, and more.

The Integrated Care Social Care Network links individuals and families with essential resources that address the social factors impacting health, such as housing, employment, food, and transportation.
Integrated Care is a regional initiative that unites community-based organizations with health centers and behavioral health providers to form a coordinated system aimed at addressing the underlying causes of health inequities. By integrating social supports with health services we work to enhance the overall well-being of Medicaid recipients across Western NY.

To submit a referral to Integrated Care for an individual click here. The individual will be contacted by a Navigator to provide more information and begin screening and process to determine eligibility for services.
